  • Perioperative Pharmacotherapy, An Issue of Anesthesiology Clinics

    • 1 Edición
    • Volumen 28-4
    • Alan David Kaye
    • Inglés
In this issue of the Anesthesiology Clinics, there are many exciting pharmacology reviews giving us a glimpse into future practice. It begins with a definitive review by Drs. Pamela Palmer and Ronald Miller on current and developing methods of patient-controlled analgesia. There are five focused reviews on drugs designed and utilized in pain management and drug addiction: buprenorphine, acetaminophen, an oral sufentanil preparation, and tapentadol. These reviews are timely because so many new drugs have come to market to modulate and to better target pain and inflammation. Likewise, there has been an explosion in use of a number of anticoagulants that modulate the coagulation cascade. Two reviews provide a clear understanding of how many of these newer agents work, discussion of the new recombinant activated factor VII (Novo7®), and guidelines for these drugs in clinical anesthesia and pain practice. Evolving understanding and clinical use of dexmetomidine, in the OR, ICU, and peripheral sites has reshaped the practice of sedation for a large population of patients. These topics describe highly clinically relevant perioperative agents. There are also reviews on pharmacological options in the management of patients with hypertension, cardiac disease, and new cardiac anesthesia agents in the OR and ICU. There is a special review on the role of statins in cardiac anesthesia, a very exciting and evolving topic. Also reviewed is levosimendan, a calcium sensitizer utilized for acutely decompensated congestive heart failure and shown to reduce cardiac troponin release after cardiac surgery. Lastly, the future of induction and inhalation agents and structure-based modifications are examined.

  • Musculoskeletal Trauma

a guide to assessment and diagnosis
    • 1 Edición
    • Maryann Hardy + 1 más
    • Inglés
Musculoskeletal Trauma: a guide to assessment and diagnosis is a clinically focused text following patient journey from presentation to diagnosis in musculoskeletal trauma. An understanding of this process is essential to ensure that clinical assessment and diagnostic examinations inform decision making and effectively and efficiently optimize patient care. This text is particularly timely given the increasing blurring of professional boundaries within the emergency care setting and need for practitioners recognise the contribution of all professions to the patient care pathway and enhance interdisciplinary communication. Written by an experienced multiprofessional clinical team, the book systematically guides the practitioner from musculoskeletal injury mechanisms, through clinical presentation and assessment, radiographic imaging and interpretation, to the formulation of a diagnosis. It is a coherent, logical and informative guide to contemporary practice for all emergency health care practitioners. An introduction is provided to the principles of clinical assessment and radiographic technique. Radiographic images are provided alongside photographs of radiographic positioning to enhance understanding of anatomical relationships and image appearances. In addition, images of both clinical presentation and radiographic features of trauma are provided to enhance understanding of clinical signs and symptoms of trauma and support accurate decision-making. Whilst conventional imaging remains the mainstay for early diagnosis in the emergency department, the contribution of other imaging modalities (in particular CT, MRI & ultrasound) are explored and illustrated with appropriate clinical cases.

    • George Thomas Lewith + 2 más
    • Inglés
The use of complementary therapies is exploding, increasing the pressure to establish a rigorous science to support its practice. Clinical Research in Complementary Therapies: Principles, Problems and Solutions provides students with the tools they need to research complementary and integrative medicine (CIM) and so fill this gap. Essential for both undergraduate and postgraduate students, this second edition is significantly updated and enhanced. Part 1 deals with research strategies and methods, explaining the major types of clinical research in CIM and how these inter-relate. New chapters are included on whole systems research, qualitative research and questionnaire development. Not all therapies can be treated the same way nor channeled through the signal process of randomized controlled trials. Therefore, detailed description of mixed methods approaches including observational, qualitative, cost-benefit and comparative effectiveness research are described. Part 2 deals with specific complementary therapies and how they are invested by experts in each field. The book analyses the key questions asked and the controversies debated in complementary medicine research and offers clear and innovative guidance for answering these questions. FEATURES • Provides an overarching synthesis of methods in CIM and how they are to be used collectively including the role of comparative effectiveness research• Suggests both general and specific factors which need to be considered in assessing or planning complementary therapy research • Pinpoints aspects of research which are different in orthodox research and complementary therapy research • Reviews the types of research carried out in specific complementary therapies and analyses issues which arise • Includes information on measuring the economic cost and benefits of complementary medicine, clinical audit and the role of placebos use • Builds upon recent research results, looks at the lessons these provide for all complementary therapies and suggests key issues to address in future research.
