    The effects of hand burn injuries can be critical to quality of life and crucial to long-term functional outcomes following burn injury. Since hands are at the front line of human contact, a high percentage of thermal injury involves the upper extremity and, in particular, the hand. Hand burns can vary in severity from shallow burns requiring local wound care and aggressive range of motion therapy to complex wounds requiring repair of joints, tendons, and other soft tissue. Historically, given the poor survival associated with severe burn injury, many patients with hand burns simply did not survive the acute phase of treatment. If patients survived the systemic insult of burn injury, often the hands were neglected relative to more extensive areas on the trunk. However, with the widespread use of early excision and grafting, as well as great advancements in critical care, survival following thermal injury has become the rule rather than the exception. Therefore, emphasis in burn care has shifted towards optimizing the functional and psychosocial outcomes of those that survive their injury. Accordingly, optimal management of hand burns has received increasing attention given the critical importance of hand recovery to long-term outcome. In this volume of Hand Clinics, experts in burn care present an overview of pediatric and adult hand burn management — including shallow burns, as well as complex injuries from deep thermal burns or electrical injury. In addition, chapters on pathphysiology of scar, burn hand rehabilitation and assessing outcomes of hand injury emphasize critical concepts in achieving optimal hand function after injury.

    Benign tumors of the base of the skull, such as acoustic neuroma, glomus jugulare and meningiomas will commonly cause symptoms referable to the realm of an otolaryngologist. Specifically, the symptoms of these types of tumors may include hearing loss, dizziness, tinnitus, facial numbness, difficulty swallowing and/or facial paralysis. For this reason, otolaryngologists are the physicians most frequently involved in the care of these patients. More importantly, otolaryngologists are the physicians best suited to treat the majority of complications associated with treatment of these types of tumors. Until the last past decade or so, traditional microsurgery has been the main treatment for benign skull base tumors. In the last few years, radiotherapy and radiosurgery have been used with increasing frequency to treat these lesions. The North American Skull Base Society predicts that by the year 2010 a greater percentage of acoustic tumors will be treated with radiosurgery than by traditional microsurgery. In order to prepare for this paradigm shift in treatment, otolaryngologists must become familiar with the different types of radiation treatments as well as the potential complications associated with radiation treatment for benign skull base tumors.

    The understanding of hemostasis physiology has been considerably advanced by models of kinetics and by the complicated interplay of cells and soluble coagulation factors. How this physiology is currently, or will eventually, be reflected in clinical laboratory testing is the subject of this monograph; this information is key for laboratorians to implement physiologic concepts into practical data and for clinicians to understand the basis of and therefore correctly treat hemostatic disease. Many thousands of patients present with bleeding disorders or bleeding complications of other diseases, and millions of individuals suffer the morbidity and mortality of thromboembolic complications. How laboratory testing confronts the challenge of predicting different hemostatic risks and guiding therapy is the critical subtext of the chapters in this monograph. New developments in hemostasis physiology have identified thrombin as an important, if not central, coordinator of hemostatic function and thus a target for measurement to assess hemostatic function and risk. Whether such measurements as endogenous thrombin potential or thromboelastography will accurately predict and/or quantitate global hemostatic function is an important question. Modeling the clinical risks of bleeding or thromboembolism currently uses the laboratory presence or absence of particular risk factors, but our clinical understanding of risk appears to more closely approximate a dynamic model, even within individual patients. Therefore, testing for platelet dysfunction or comprehending the functional implications of a lupus anticoagulant may rely on our evolving comprehension of hemostasis phyisology and perhaps require more sophisticated interpretation of clinical predictors of hemostatic risk. This monograph aims to shed some light in these areas and promote investigation of such key hemostasis issues.
