    Alcohol abuse remains a significant problem world-wide. The most commonly affected organ remains the liver with a risk of alcoholic liver disease (ALD) which can range from asymptomatic to alcoholic hepatitis to alcoholic cirrhosis. In 2010, alcohol-attributable cirrhosis was responsible for 0.9% of all global deaths. Given the sheer magnitude of alcohol abuse and alcoholic liver disease, an update on this topic is pertinent and relevant. This issue will focus on a variety of topics including alcoholism, the pathogenesis of ALD, the spectrum of ALD, the pathology of ALD, and the long-term management of patients with ALD. Alcoholic hepatitis is a particular type of ALD with a high mortality. This issue will explore the syndrome of alcoholic hepatitis, its prognostic markers, and the available therapies. Because alcohol intake can influence so many other conditions of the liver (especially hepatitis C), articles in this issue will also explore the impact of alcohol abuse on other liver conditions. In addition, several other topics such as nutritional therapy for ALD, ALD and liver cancer, and ALD and infection risk will be explored. Also, even though it is quite controversial, we will also explore liver transplant as a therapy for alcoholic liver disease including alcoholic hepatitis.

The approach for this publication in Otolaryngologic Clinics is to provide clinically relevant information in a problem-based manner that encompasses assessment to identify patients with a high risk for vascular complications, pre-operative, intra-operative and post-operative strategies to manage expected bleeding and major vessel injury during endoscopic sinus and skull base surgery for inflammatory and neoplastic diseases. Clinically relevant anatomy, physiology, pharmacology, and surgical and interventional radiology techniques to manage bleeding are outlined and an algorithm for management of major vessel injury such as internal carotid artery injury is presented. Because skull base surgery is generally undertaken as a team approach, this information is relevant to the subspecialized ENT surgeon from rhinology or head and neck specializations, to neurosurgeons, to interventional radiologists, and to neurophysiologists who monitors intraoperative cerebral and cranial nerve activity for these operations. Carl Snyderman leads this issue with Harshita Pant and has composed topics and assembled an expert group of practitioners to provide information.     Preterm neonates remain at increased risk for adverse bilirubin-related outcomes, including acute bilirubin encephalopathy relative to term infants. Yet, most vulnerable neonates are likely to benefit from the potent anti-oxidant properties of bilirubin. Evidence-based guidelines for the management of hyperbilirubinemia in preterm infants, however, are lacking. High concentrations of unconjugated bilirubin can cause permanent neurologic damage in infants, evident through magnetic resonance imaging of chronic bilirubin encephalopathy or kernicterus. There is a growing concern that exposures to even moderate concentrations of bilirubin may lead to subtle but permanent neurodevelopmental impairment referred to as bilirubin-induced neurologic dysfunction. Our current use of phototherapy to decrease bilirubin loads and its potential photo-oxidant properties is a biological conundrum that has been questioned in the use of phototherapy for very low birth weight neonates. In this issue of Clinics in Perinatology, we provide updates on the current understanding of the biology, mechanisms of increasing bilirubin load due to hemolysis, decreased bilirubin binding capacity and glucose-6-phosphate dehydrogenase deficiency, as well as clinical strategies to operationalize the thresholds for hyperbilirubinemia interventions in preterm infants.

    For many years, physicians and the public assumed that the discovery of new antimicrobial agents would outpace the ability of bacteria to mutate and develop drug resistance. Yet the development of new antibiotics has not kept up with bacterial evolution, especially since the late 1990’s. At that time a multitude of pharmaceutical companies abandoned antibiotic research because of strong economic disincentives. For example, it is challenging for these companies to recuperate the investment (typically in the hundreds of millions of dollars) made in developing a new antibiotic, which is typically prescribed for a few days, compared to drugs that treat chronic conditions like heart disease or mental illness. This situation has led the U.S. federal government to take a more active lead in addressing antibiotic resistance. Recently, the White House announced an action plan that includes improving surveillance, developing better diagnostic tools, accelerating drug development, and improving global coordination of antibiotic resistance issues. Equally important is the $1.2 billion dollars that has been pledged to fund these efforts. While we await the implementation of new policies, this issue of Infectious Disease Clinics of North America brings together leading authorities in the field of antibiotic resistance who discuss current issues including antibiotic stewardship, the changing role of the microbiology laboratory in determining antibiotic resistance in gram-negative pathogens, the continuing spread of metallo-β-lactamases...     Obstetrician Gynecologists are frequently responsible for management of the primary care needs of their patients. A survey performed in 2005 found an estimated 37% of, non-pregnant patients, relies on gynecologists for routine primary care. The same study found that almost a quarter of gynecologists reported they needed additional primary care training across a broad set of medical topics (Acad Med. 2007; 82:602–607). The impetus for training in primary care skills is increasing. In response to language in the Affordable Care Act, the Institute of Medicine developed a report on clinical preventative services necessary for women (Clinical Preventative Services for Women: Closing the Gaps IOM. 2011; also Current Opinion in Obstetrics and Gynecology 2011, 23:471–480). The US Department of Health and Human services has adopted these IOM recommendations and, as a result, health plans are required to include these services. While initiatives such as the American Congress of Obstetricians and Gynecologists’ Well-Woman Task Force and recent cross-specialty ACOG educational collaborations have begun to address supplemental educational needs, additional resources covering key primary care topics are necessary.
